- Context
A Canadian Provincial Health Ministry was exploring new primary care models, focusing on providing healthcare that is whole-person, prevention-focused, and delivered by interprofessional teams. In particular, they were interested in providing high-quality primary care and urgent care services in one location.
- Our Approach
- Conducted a jurisdictional review on similar primary care models.
- Worked with a primary care group on a proposal for innovative model of care.
- Interviewed various provincial health agencies to understand existing policies and to explore how best to support the proposed model.
- Conducted community engagement sessions with the public, healthcare leaders, and healthcare providers.
- Reviewed health human resource requirements to implement the proposed model.
- Developed a detailed business plan that included staffing models, funding framework, capital investment, and an implementation plan.
- Outcome
The Ministry of Health approved the business case for the new integrated primary care model. It is anticipated that the prevention-focused model of care will reduce overall health system costs through improved health outcomes, lower prevalence of chronic disease, and lower utilization of higher acuity services.
